How do you become a commissioned officer in the military as opposed to a NCO?

I've never understood how someone becomes a CO instead of an NCO. I understand that once you enlist, you go to basic, then you become a private. How do you move from those ranks up to the metal bars though? What does a CO do differently in the process than an NCO to go directly to 2LT instead of starting at private?
