How much does a rogers SIM card cost?

I already have a phone and a SIM card. My friend was messing around with my phone and entered the PUK code wrong too many times. This fried the SIM card and now I have to get a new one. How much would this cost? And will i still have all my contacts and information that was saved on my previous SIM card? Will my information be transferred over?
